If you rely on dodging most of the attacks, transcendence can prevent you from dying in one/two hits later in the game. You can also use it to help counter the effects of shaped glass. But of course, if you are trying to tank the damage by healing (and especially if you are playing engineer), it's not worth getting.

The Transcendence is a lunar item in Risk of Rain 2. It increases the holder's maximum health by 50% (+25% per stack), but replaces all but 1 of their health with shields, which regenerate over a period of 2 seconds after avoiding damage for 7 seconds. Any health gained afterwards, such as by leveling up or picking up health-increasing items, will also be increased and converted into shields ... The poison damage is 1% of the enemy's max HP per second, clamped between 1 and 50 times your damage. Anything that effects your character's damage stat (Glass, Tonic, etc) will effect the clamp range. Anything that increases damage dealt to an enemy (Shattering Justice, Focus Crystal, etc) will increase the poison damage.
