I’ve found it a couple of times already in boss level dungeon chests. If you do the miscellaneous objective to kill the mad blacksmith and learn how to make madness and amber gear, you can get a guaranteed piece of madness ore on the altar table. Then you can do a duplication glitch to get more of it or something. Nice!

Well, Amber spawns in the main dungeon associated with the quest, then gets added to Khajiit Caravans afterward. Madness can spawn in boss chests, and also Khajiit Caravans afterward.
