Check your audio settings. If you have surround sound enabled but don't actually have a surround speaker set up, the game tries to put the voice acting channel to a speaker that doesn't exist. If you mean in my PC audio settings, I checked the device and there is no surround sound enabled on them. If you in the game I dont see the option for it?

When you connect a PS4 controller, it sometimes tries to set the controller as the sound output. Make sure to go into windows and change back to the sound device you want after plugging it in. Sound came through from FFXIV no problem, and switching the audio default back to speakers didn't crash it. I stuck with speakers for the past couple of hours (once I got back in the game through the stupid-long queue).
